A beautifully placed dermal filler treatment should leave you looking refreshed, not obviously treated. The hours and days afterwards play a part in that outcome. This dermal filler aftercare guide explains how to protect the treated area, manage expected swelling and recognise the rare signs that need prompt medical advice.

Dermal fillers are injectable gels, most commonly made from hyaluronic acid, used to restore volume, soften lines or refine facial balance. Even when treatment is gentle and precise, the skin and underlying tissue need time to settle. A little patience is often the most valuable part of aftercare.

The first few hours after dermal filler

Your practitioner will give you advice tailored to the areas treated, the amount of filler used and your medical history. Follow that guidance first, particularly if it differs from general advice online.

For the rest of the day, keep your hands away from the injection points unless you have been specifically shown how and when to massage an area. It can be tempting to keep checking a small lump or unevenness, but pressing the tissue repeatedly may increase swelling and discomfort. Filler should only be massaged if your practitioner advises it.

Avoid touching the area unnecessarily and attend with clean skin if a review is required. If you need to apply make-up later that day, use clean brushes or applicators and apply it very gently. Some practitioners may prefer you to leave the skin bare for a set period, especially if there are several injection points.

Drink water, eat normally and plan a quiet evening where possible. Hyaluronic acid fillers attract water, but drinking excessive amounts will not make the result settle faster. Good hydration simply supports your general wellbeing and skin comfort.

What to avoid in the first 24 to 48 hours

For a day or two, the aim is to limit anything that increases blood flow, heat, pressure or irritation around the treatment area. This does not mean you need to put life on hold, but it is sensible to be cautious while early swelling settles.

Avoid strenuous exercise for 24 to 48 hours. A gentle walk is fine, but a high-intensity gym session, long run or hot yoga class can make swelling and bruising more noticeable. It is also best to avoid alcohol for at least 24 hours, as it may contribute to flushing, swelling and bruising.

Skip saunas, steam rooms, hot tubs and very hot baths for 48 hours. Keep showers comfortably warm rather than hot. Direct sun exposure is also best avoided initially, particularly if you are prone to bruising or pigmentation. Once the injection points have settled, apply broad-spectrum SPF as part of your usual skincare routine.

Try not to put prolonged pressure on the treated area. If you have had cheek, jawline or temple filler, avoid lying face-down to sleep. After lip filler, avoid kissing, using straws and significant pressure to the lips for the first day or two. These are temporary adjustments, not long-term restrictions.

It is usually wise to postpone facials, facial massage, gua sha, microcurrent devices and other aesthetic treatments near the treated area for around two weeks, unless your practitioner tells you otherwise. Dental treatment can also involve sustained pressure and inflammation around the face, so discuss the timing of planned dental work before or after filler treatment.

Managing swelling, tenderness and bruising

Mild swelling, tenderness, redness, pinpoint bleeding and small bruises are common after dermal filler. Lips tend to swell more than other areas and can look fuller than expected during the first few days. Cheek and chin filler may feel firm initially, while under-eye treatment requires especially careful assessment because this area can be slower to settle.

A clean, cool compress can be used gently for short periods to ease discomfort. Do not apply ice directly to the skin and do not press firmly. Keep the compress wrapped in a clean cloth, and use it intermittently rather than continuously.

If you need pain relief, paracetamol is generally the preferred option for most people. Unless prescribed for a medical reason, avoid anti-inflammatory medicines such as ibuprofen and aspirin immediately after treatment because they can make bruising more likely. Never stop prescribed medication without speaking to the clinician who manages it.

Bruising can take a week or two to fade, depending on the area treated and your individual tendency to bruise. If you have an event coming up, allow more time than you think you may need. Concealer can usually be used once the skin has settled, but your result will look its most reliable after swelling has gone down.

When will your filler look settled?

Most early swelling improves within 48 to 72 hours. However, filler does not always reveal its final appearance immediately. Allow around two weeks for most areas to settle fully before judging the result or deciding whether you would like a review.

This matters because treatment can look slightly uneven while one side is more swollen than the other. It also matters because a natural-looking result is often deliberately subtle. More filler is not automatically better. The right amount depends on your facial structure, skin quality, the product used and the result you discussed at consultation.

If a planned review is part of your treatment plan, attend it. Your practitioner can assess the result in person and decide whether no further treatment, a small adjustment or simply more settling time is appropriate. A considered approach protects the balanced, recognisable result most people want.

When to contact your practitioner

Most post-treatment concerns are minor, and a quick message or review can provide reassurance. Contact your practitioner promptly if swelling is worsening rather than improving after the first couple of days, if you develop increasing redness or warmth, or if you notice a painful lump that concerns you.

You should also get in touch if you have a history of cold sores and develop tingling, burning or blisters around the lips after treatment. Antiviral treatment may be needed, particularly following lip filler.

A medically led clinic should provide clear aftercare contact details before you leave. Do not rely on social media advice or attempt to correct a concern yourself with massage, heat or additional products.

Urgent warning signs after filler

Serious complications are uncommon, but they require immediate action. Contact your practitioner urgently if you experience severe or increasing pain, skin that becomes unusually pale, white, dusky, grey, blue or mottled, or skin that feels cold around the treated area. These changes can indicate a problem with blood supply and must not be watched overnight.

Visual disturbance, including blurred vision, loss of vision, severe headache or weakness, is a medical emergency. Seek emergency help immediately by calling 999 or attending A&E, while also contacting your treating practitioner. Fast assessment is essential.

Skincare and make-up after treatment

Keep your skincare gentle for the first day. Avoid active products that can sting or irritate freshly injected skin, including strong retinoids, exfoliating acids and scrubs. A mild cleanser, simple moisturiser and SPF are usually enough once the skin is comfortable.

You do not need to buy an entirely new routine after filler. In fact, the best approach is often to avoid introducing unfamiliar products while the area is settling. If you are using prescribed skincare or have recently had laser treatment, ask for individual advice, as the timing may need adjusting.

Planning future treatments thoughtfully

Dermal filler is not a one-size-fits-all treatment, and neither is the maintenance schedule. Some areas may hold their result for several months, while others can last longer. Metabolism, movement, product choice and the treatment area all influence longevity.

Rather than booking repeat treatment by the calendar alone, arrange a review when you feel the original concern is beginning to return. This supports a less-is-more approach: maintaining facial harmony over time without chasing a permanently just-treated appearance.

Good aftercare is not about being overly cautious. It is about giving your face the quiet recovery time it needs and knowing that expert support is available if something does not feel right. A careful treatment plan, realistic expectations and a trusted medical practitioner remain the foundation of results that still look like you.
